Before Your Visit

If you are a new patient to Ely Surgical Associates you can complete your new patient form prior to your appointment.  Please print the new patient form below, complete it and bring it with you to your appointment.

Please remember to bring with you, along with your new patient form, your insurance cards, co-payments and a list of your current medications.

After Your Visit

Post-op and Discharge Instructions

Outpatient Surgery:

• Do not lift more than 5 lbs. for 7-10 days post surgery.
• Do not drive or operate machinery for 3-5 days post surgery.
• Do not bathe or shower for 48 hours after surgery.
• Change the dressing(s) as necessary.  You may use band aids or small gauze pads for dressing material.

Inpatient surgery:

• Do not lift more than 10 lbs. for 3-4 weeks after surgery.
• Do not drive or operate machinery for 10-14 days after surgery.
• You may shower or bathe at 7-10 days as long as you do not have any open wounds remaining.
• Change the dressing(s) as outlined prior to your discharge.

Discharge Instructions:

• Do not lift more than 5 lbs. for 7-10 days after surgery.
